Topic: crazy diamond help

ok , im trying to learn a chord sequence of a guy playing shine on crazy diamond on youtube

ive got ,i think as far as , not counting the lead bits 

C - C - F -Gm -

D# -D -Gm -

Cm -Gm - the last chord resolves to ?????? 
 
does any 1 know of any tab , with chords & lyrics any where ,
all the ones i can find are all well , not for me , i know thats no excuse but the chords above 
the lyrics help me loads , cheers paul

update .  hi again could some 1 possibly tell me the exact fingerings of these last set of chords
              i need to know , would really appreiciate it thanx  , all except Eb Dm Gm
