immediately after first down stroke

For this chord the middle finger presses both 5th and 6th string.
4th string is hammered on immediately after first stroke. The 1st string
is accented on some upstrokes or muted. Mute by lightly touching the 1st
string with your pinky on unaccented strokes and pressed during accented
upward strokes.

The 6th and 4th strings of the D chords are hammered on after first stroke.
